This is the United States and I think handled by HIPAA.
My dentist died late last year. Recently, I received a letter that his family sold the practice and has already transferred all the data to another dentist.
I Want You Back My Records - What are my rights, what HIPAA is that?

You should be able to file its application for a new dentist who has acquired the practice. Stop by the office and talk to the reception. I know in the office I worked in everything you do is a form and a copy of the documents you need to take, or they can fax it to the office from other dentists.
